Terms of Service

Welcome to Formula Health. These Terms of Service (“Terms”) form a binding agreement between you and Formula Health (“Formula Health,” “we,” “us”) governing your access to and use of the Formula Health website, mobile application, CORA AI assistant, and related services (collectively, the “Service”). By creating an account or using the Service you agree to these Terms.

1. Wellness Service, Not Medical Care

Formula Health is a consumer wellness platform. The Service, including CORA, provides educational and lifestyle information based on the data you supply. It is not medical advice, diagnosis, or treatment. Always consult a qualified health professional for medical questions. In an emergency, call your local emergency services.

If the Service connects you with Independent Physicians, those physicians are independent licensed professionals who deliver care under their own practices. Any clinical relationship is between you and the Independent Physician, not Formula Health.

2. Eligibility & Accounts

You must be at least 18 years old (or the age of majority in your jurisdiction) to use the Service. You are responsible for keeping your login credentials, including any magic codes, secure. You agree to provide accurate information and to notify us promptly of any unauthorized use of your account.

3. Subscriptions, Billing, and Cancellation

  • Plans are billed in advance on a monthly or annual basis through our payment processor, Stripe. By subscribing, you authorize recurring charges until you cancel.
  • You may cancel anytime from the app or by contacting support@formula-health.com. Cancellation ends future renewals; access continues through the current paid period.
  • Pricing, including any introductory pricing such as “First 1000” offers, is shown at checkout and may change with notice for future billing periods.
  • Refunds are handled on a case-by-case basis and are not guaranteed except where required by applicable law.
  • Independent Physician consultations may be billed separately by the physician's practice and are subject to that practice's own terms.

4. Acceptable Use

You agree not to:

  • Use the Service for any unlawful, harmful, or fraudulent purpose.
  • Attempt to interfere with, reverse engineer, or compromise the security of the Service.
  • Submit false health information, impersonate another person, or share your account.
  • Use the Service to provide medical care to anyone other than yourself.
  • Scrape, copy, or use Service content for competitive purposes without permission.

5. User Content and CORA Inputs

You retain all rights to content you submit (“User Content”), including data sent to CORA. You grant Formula Health a worldwide, non-exclusive, royalty-free license to host, store, and process User Content solely to operate the Service for you. You are responsible for the accuracy of what you submit.

6. CORA and AI-Generated Output

CORA produces educational responses generated by AI. AI output may be incomplete or inaccurate. Do not rely on CORA as a substitute for professional medical, legal, or financial advice. We do not guarantee any specific result from your use of the Service.

7. Third-Party Integrations

The Service can connect to third-party providers (Apple Health, Whoop, Oura, Google Health, and others) with your permission. Your use of those providers is subject to their own terms. We are not responsible for third-party content or services.

8. Intellectual Property

The Service, including the Health Optimization Model, the HOM Score, CORA, all software, and our trademarks and content, are owned by Formula Health or our licensors and are protected by intellectual-property laws. You receive a limited, non-transferable license to use the Service in accordance with these Terms.

9. Disclaimers

THE SERVICE IS PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING ANY WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SERVICE WILL BE UNINTERRUPTED, ERROR-FREE, OR PROVIDE ANY PARTICULAR HEALTH OUTCOME.

10. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, FORMULA HEALTH'S TOTAL LIABILITY ARISING OUT OF OR RELATING TO THESE TERMS OR THE SERVICE IS LIMITED TO THE GREATER OF (A) THE AMOUNTS YOU PAID US IN THE 12 MONTHS BEFORE THE CLAIM, OR (B) USD 100. WE ARE NOT LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR EXEMPLARY DAMAGES.

11. Indemnification

You agree to indemnify and hold Formula Health harmless from claims, damages, and expenses arising out of your use of the Service or your violation of these Terms.

12. Apple App Store / Google Play Terms

If you obtain the Formula Health app through the Apple App Store or Google Play, additional terms from those providers apply. Apple is not a party to these Terms and is not responsible for the Service or any claims relating to it.

13. Termination

We may suspend or terminate your access if you violate these Terms or for security or legal reasons. You may stop using the Service at any time. Sections that by their nature should survive termination will survive.

14. Governing Law and Disputes

These Terms are governed by the laws of the State of Delaware, USA, without regard to conflict-of-laws rules. Any dispute will be resolved in the state or federal courts located in Delaware, except where applicable consumer-protection laws require otherwise.

15. Changes to These Terms

We may update these Terms. Material changes will be communicated through the Service or by email. Your continued use after changes take effect means you accept the updated Terms.
